Our short-term stay skilled nursing team specializes in providing compassionate, around-the-clock care to those who have recently been released from the hospital. At eliseo, our short-term rehabilitation center is designed with your comfort and recovery in mind. With 50 private rooms, you or your loved one can heal in a quiet, restful environment that promotes peace of mind and personalized care.
Getting Released from the Hospital Soon?
Your hospital care team will assess you or your loved one’s condition and determine the appropriate level of care upon discharge. They will also provide you with a list of available facilities to be discharged to.  You have a choice in where you are discharged! If eliseo is not on that list, you may request that a referral be sent to eliseo to get the release and transfer process started. Once you are medically cleared to transfer, the hospital will coordinate with us to get you to eliseo and settled in our Health Center.Â
