Considering Long-Term Care at eliseo
If you are wanting to transfer directly from a hospital, the care team at the hospital will assess you or your loved one’s condition and determine the appropriate level of care upon discharge. They will also provide you with a list of available facilities to be discharged to.  You have a choice in where you are discharged! If eliseo is not on that list, you may request that a referral be sent to eliseo to get the release and transfer process started. Once you are medically cleared to transfer, the hospital will coordinate with us to get you to eliseo and settled in our Health Center.
If you are wanting to transfer directly from home, please call our Admissions team and they can describe the steps you need to take to get you or your loved one to the Health Center.
